Change of Name Deed

Our change of name deeds are guaranteed to be accepted by the UK passport office. We provide a physical copy sent directly to your door, which will be accepted by the passport office.

Once you have made your purchase, you will simply have to fill in the blank spaces left on the form with a pen, we will also provide you with a template to assist you with this. Please note these change of name deeds are for people ages 16 and over. For children change of name deeds, all parties with parental responsibility will need to sign (e.g. Mother & Father if named on Birth Certificate)
